Visa Pages
Do you need to add visa pages to your currently valid U.S. passport book?
U.S. passport holders 16 and older must personally sign the application form, but do not need to come to the Embassy personally to request new pages.
For children under 16, one parent needs to sign the application form DS-4085. On the form, the parent should indicate his/her relationship next to the signature on the form. Though not required, the consul may ask to see an original or certified copy of the child’s birth certificate showing both parents names. The child does not have to appear at the Embassy.
Note: In general, you are not allowed to add more than two supplement visa page inserts. If you already have two inserts in your passport, you will be required to apply for a new passport.
- Complete Form DS-4085. Either enter your information online and print Form DS-4085 OR print out the blank form and then complete using black ink on one-sided pages. Failure to provide information requested on this form, including your social security number, may result in significant processing delays and/or the denial of your application.

- The fee for adding visa pages is $82 payable in dollars or the equivalent in Kenyan Shillings, USD or international credit card (VISA, MasterCard, Dinners, American Express, and Discover).
