ATTENTION REGISTERED VIRGINIA VOTERS!

Declaration/Affirmation witness requirements:
Registered Virginia voters who have already requested but not received their state ballot, may use the FWAB as a backup ballot. The Voter
Declaration/Affirmation must be witnessed with signature and date (witness address not required).

Registered Virginia voters who have not requested their ballot may use the FWAB as a simultaneous absentee ballot request and an absentee ballot as long as it is received 5 days before the November 4 election. The FWAB must contain the residence address where the voter is registered in Virginia, the voter's current military or overseas address, and the voter's and witness' signature and date. The witness must provide his or her printed name and address in Block 6.
